Conformidade entre os cuidados de enfermagem e o feixe de intervenções para a prevenção da infeção do local cirúrgico

Enquadramento: Segundo o Regulamento de Competências Específicas do Enfermeiro Especialista em Enfermagem Médico Cirúrgica na área de enfermagem à pessoa em situação crítica, uma das suas competências é a maximização, prevenção, intervenção e controlo da infeção e de resistência a Antimicrobianos. As infeções associadas aos cuidados de saúde são um problema de saúde pública, tendo em conta a elevada morbilidade/mortalidade e custos inerentes associados. A Direção Geral de Saúde, no seu Relatório do Inquérito de prevalência de ponto em hospitais de agudos em Portugal 2017, refere que a Infeção do Local Cirúrgico, foi a 3ª maior causa de infeção (18.5%).
Objetivos: Descrever o percurso para obtenção de competências na área da Enfermagem Médico-Cirúrgica na área de enfermagem à pessoa em situação crítica e desenvolver a componente investigação com a avaliação da conformidade entre os cuidados de enfermagem e o Feixe de Intervenções para Prevenção da Infeção do Local Cirúrgico.
Métodos: Foi realizado um relatório final com uma componente clínica resultante dos estágios realizados e uma de investigação através de um estudo descritivo transversal, realizado numa amostra não probabilística, constituída por 65 enfermeiros, de unidades hospitalares privadas com o objetivo de identificar se existe conformidade entre os cuidados de enfermagem e o Feixe de Intervenções para Prevenção da Infeção do Local Cirúrgico e quais as variáveis sociodemográficas que podem influenciar essa conformidade. O Instrumento de Colheita de Dados foi: “Questionário de Conformidade entre os cuidados de enfermagem e o Feixe de Intervenções para Prevenção da Infeção do Local Cirúrgico”, elaborado por Almeida & Gama com base no Feixe de Intervenção da Direção Geral de Saúde.
Resultados: A amostra é constituída na sua maioria por mulheres (69,2%%), com média de idades de 35,29, 15,4% com o título especialista e 7,7% com o grau mestre. Face aos resultados obtidos, verificou-se que a maioria dos enfermeiros prestam cuidados em conformidade com o Feixe de Intervenções e que as variáveis Sexo, Formação Académica, Tempo de Experiência Profissional, Serviço em que exerce funções, conhecimento do Feixe, influenciam a conformidade dos cuidados de enfermagem.
Conclusão: O estágio permitiu desenvolver competências científicas, técnicas e humanas na prestação de cuidados especializados à pessoa em situação crítica, desenvolvendo uma prática baseada na evidência científica. Também foi possível desenvolver competências na área investigação prática e perceber que dos resultados obtidos, emerge a necessidade de incentivar os enfermeiros a investir na sua formação profissional e na realização de ações de formação periódicas sobre a temática.

Abstract Background: According to the Regulation of Specific Skills of the Specialist Nurse in Medical-Surgical Nursing in the area of nursing for people in critical situations, one of their skills is the maximization, prevention, intervention and control of infection and resistance to Antimicrobials. Healthcare-associated infections are a public health problem, given the high morbidity/mortality and associated costs. The General Directorate of Health, in its Report on the Point Prevalence Survey in Acute Hospitals in Portugal 2017, states that Surgical Site Infection was the 3rd biggest cause of infection (18.5%). Objectives: To describe the path to obtaining skills in the area of Medical-Surgical Nursing in the area of nursing for people in critical situations and to develop the research component with the assessment of compliance between nursing care and the Bundle of Interventions for the Prevention of Surgical Site Infection. Methods: A final report was produced with a clinical component resulting from the internships carried out and a research component through a cross-sectional descriptive study, carried out in a non-probabilistic sample, composed of 65 nurses, from private hospital units with the aim of identifying whether there is compliance between nursing care and the Bundle of Interventions for the Prevention of Surgical Site Infection and which sociodemographic variables can influence this compliance. The Data Collection Instrument was: “Questionnaire on Compliance between Nursing Care and the Bundle of Interventions for the Prevention of Surgical Site Infection”, prepared by Almeida & Gama based on the Intervention Bundle of the State Health Department. Results: The sample is composed mostly of women (69.2%%), with an average age of 35.29, 15.4% with a specialist title and 7.7% with a master's degree. Given the results obtained, it was found that the majority of nurses provide care in accordance with the Intervention Bundle and that the variables Sex, Academic Background, Length of Professional Experience, Service in which they work, knowledge of the Bundle, influence the compliance of nursing care. Conclusion: The internship allowed the development of scientific, technical and human skills in the provision of specialized care to people in critical situations, developing a practice based on scientific evidence. It was also possible to develop skills in the area of practical research and to understand that from the results obtained, there is a need to encourage nurses to invest in their professional training and in carrying out periodic training actions on the subject.
