Abstract Introduction: Immediate caregiver response in pediatric emergencies is crucial for the child’s prognosis. In Portugal, the scarcity of studies systematically assessing parental literacy in first aid and cardiopulmonary resuscitation in pediatric emergency settings justifies this research. Objective: To critically reflect on the clinical training pathway during internships, and to assess the level of knowledge of parents and caregivers regarding pediatric first aid and cardiopulmonary resuscitation, identifying associated factors and perceived confidence to act in emergency situations. Methodology: Observational, descriptive-analytical, and quantitative study. The non-probabilistic convenience sample included 100 parents and caregivers who attended a pediatric emergency department of a Local Health Unit. Data were collected through an anonymous, self-administered digital questionnaire assessing sociodemographic data, previous training, prior experiences, and knowledge of pediatric emergency situations. Results: High levels of correct responses were observed in basic emergency activation procedures, namely knowledge of the emergency number (99%) and the approach to unconsciousness (98%). Significant gaps were identified in more technically complex procedures: only 50% correctly answered the compression-to-ventilation ratio in pediatrics, and 34% correctly identified the appropriate management of caustic substance ingestion. Regarding confidence to act, 46% of participants reported feeling confident to perform CPR on their own child, contrasting with 95% who considered training in this area to be very important. Conclusion: The findings highlight insufficient knowledge in life support procedures and limited confidence to act in pediatric emergency situations, reinforcing the need for practical and accessible educational interventions promoted by specialist nurses. Keywords: first aid; cardiopulmonary resuscitation; health literacy; parents; child.
