Introdução: Atualmente, perante a evolução técnica e científica em saúde observa-se, um aumento da complexidade dos cuidados prestados. É essencial a formação específica e especializada, dos profissionais de saúde forma a responder às demandas atuais dos cuidados em saúde. Para o desenvolvimento de uma prática especializada em Enfermagem Médico-Cirúrgica, nomeadamente na área de enfermagem à pessoa crítica, é essencial não só a aquisição de competências científicas e técnicas, mas também de competências relacionais. A investigação em Enfermagem é fundamental para o desenvolvimento de uma prática profissional segura e promotora da melhoria dos cuidados em enfermagem. Objetivo: Demonstrar as competências específicas adquiridas em Enfermagem Médico-
Cirúrgica na área da pessoa em situação crítica. Avaliar qual a qualidade de sono do doente cirúrgico. Métodos: Este trabalho encontra-se dividido em dois capítulos. No primeiro capítulo são descritas as atividades desenvolvidas durante os estágios clínicos realizados em contexto de urgência e cuidados intensivos, bem como a análise e reflexão crítica sobre as mesmas e o seu contributo para a aquisição e o desenvolvimento de Competências Comuns e Específicas do Enfermeiro Especialista em Enfermagem Médico-Cirúrgica. No segundo capítulo é apresentado o estudo descritivo-correlacional de matriz transversal, realizado com 150 doentes internados para procedimento cirúrgico, num centro hospitalar português. Resultados: A realização dos estágios clínicos permitiu, o desenvolvimento de um conjunto de competências na área do cuidado à pessoa em situação crítica no que diz respeito a: i) prevenção, intervenção e controlo de infeção; ii) responsabilidade profissional, ética e legal e iii) melhoria contínua da qualidade e gestão dos cuidados prestados. A maioria dos participantes apresentou uma má qualidade de sono (PSQI>5=89.3%) durante o internamento. Conclusão: A aquisição de competência em Enfermagem Médico-Cirúrgica permite desenvolver uma prática assistencial mais especializada e competente na área da pessoa em situação crítica. Com a realização da componente de investigação, espero que esta permita uma maior consciencialização dos profissionais de saúde, para a má qualidade do sono no doente cirúrgico e assim promover o desenvolvimento de práticas clínicas que procurem abolir ou minimizar os fatores associados ao sono inadequado. Abstract Introduction: Currently, given the technical and scientific evolution in healthcare, an increase in the complexity of the care provided is observed. Specific and specialized training of healthcare professionals is, thus, essential to respond to the current demands in the area. For the development of a specialized practice in Medical-Surgical Nursing, specifically in the area of critical care nursing, it is essential not only to acquire scientific and technical skills, but also relational skills. Research in Nursing is essential for the development of a safe professional practice that can promote the improvement in nursing care. Objective: To demonstrate the specific skills acquired in Medical-Surgical Nursing in the area of patients in critical situation. To evaluate the quality of sleep of surgical patients. Methods: This work is divided into two chapters. The first chapter describes the activities developed during the clinical internships carried out in the context of emergency and intensive care, as well as their analysis and critical reflection on them and their contribution to the acquisition and development of Common and Specific Skills for a Specialist Nurse in Medical-Surgical Nursing. In the second chapter, the descriptive-correlational cross-sectional study is presented, which was carried out with 150 patients hospitalized for a surgical procedure in a Portuguese hospital. Results: The completion of clinical internships allowed the development of a set of skills in the area of care for patients in critical situation in what concerns: i) prevention, intervention and infection control, ii) professional, ethical and legal responsibility, and iii) continuous improvement of the quality and management of the care provided. Most participants experienced poor sleep quality (PSQI>5=89.3%) during hospitalization. Conclusion: The acquisition of competence in Medical-Surgical Nursing allows the development of a more specialized and competent care practice in the area of patients in critical situation. I hope that the completion of the research component will create awareness among health professionals of the poor quality of sleep in surgical patients, thus promoting the development of clinical practices that seek to abolish or minimize the factors associated with inadequate sleep.
