Resumo(s)
O presente relatório integra a componente de estágio e de investigação desenvolvidas
no âmbito do Mestrado em Enfermagem Comunitária e de Saúde Pública, tendo como
finalidade refletir criticamente sobre o percurso formativo e analisar a adequação dos
indicadores contratualizados nas Unidades de Cuidados na Comunidade (UCC), enquanto
instrumentos de suporte à avaliação da qualidade e dos resultados em saúde.
O percurso de estágio decorreu em contexto de Unidade de Saúde Pública e Unidade
de Cuidados na Comunidade, proporcionando o desenvolvimento e consolidação de
competências no domínio da avaliação de necessidades em saúde, planeamento e
intervenção comunitária, trabalho em equipa multidisciplinar e articulação com recursos da
comunidade, numa lógica de cuidados centrados na pessoa, família e comunidade.
Em paralelo, foi desenvolvido um estudo de natureza quantitativa, complementado por
análise qualitativa, centrado na perceção dos enfermeiros das UCC relativamente aos
indicadores contratualizados. De forma global, os resultados evidenciam uma leitura crítica
destes instrumentos, particularmente no que se refere à sua adequação à prática clínica, à
sua operacionalização e à sua capacidade para refletir a complexidade dos cuidados de
enfermagem em contexto comunitário.
O trabalho desenvolvido permitiu evidenciar a importância de modelos de avaliação
mais ajustados às especificidades das UCC, bem como a necessidade de integrar indicadores
sensíveis à enfermagem, promover maior participação dos profissionais e reforçar os
processos de capacitação e suporte organizacional.
Conclui-se que a evolução dos modelos de contratualização em cuidados de saúde
primários exige uma abordagem mais integrada, participativa e orientada para os ganhos em
saúde, contribuindo para a valorização da enfermagem comunitária e para a melhoria
contínua da qualidade dos cuidados prestados à população.

This report integrates the internship and research components developed within the scope of the Master’s Degree in Community and Public Health Nursing, aiming to provide a critical reflection on the educational trajectory and to analyse the adequacy of contracted performance indicators in Community Care Units (CCUs) as tools for assessing quality and health outcomes. The internship was conducted in both a Public Health Unit and a Community Care Unit, enabling the development and consolidation of competencies in health needs assessment, community-based planning and intervention, interdisciplinary teamwork, and coordination with community resources, within a person-, family-, and community-centred care framework. In parallel, a quantitative study, complemented by qualitative content analysis, was carried out focusing on nurses’ perceptions of contracted indicators in CCUs. Overall, findings suggest a critical appraisal of these instruments, particularly regarding their alignment with clinical practice, operational feasibility, and capacity to capture the complexity of community nursing care. The work highlights the need for more context-sensitive and practice-oriented evaluation models, emphasizing the integration of nursing-sensitive indicators, increased professional involvement in indicator design, and strengthened organizational and educational support mechanisms. It is concluded that the evolution of contracting models in primary health care requires a more integrated, participatory, and value-oriented approach, contributing to the recognition of community nursing practice and to the continuous improvement of care quality and health system sustainability.
