Enquadramento: Os cuidados paliativos caracterizam-se por serem cuidados
individualizados focados no bem-estar da pessoa com necessidades paliativas, promovendo
a sua dignidade e maximizando, dentro do possível, a sua qualidade de vida. A esperança
tem sido considerada como um dos elementos essenciais na vivência das pessoas com
doença oncológica. Em contexto de doença terminal, quando a cura não é possível, o enfoque
da esperança assume uma importância fundamental na forma como as pessoas doentes
vivem o período de fim da vida. De forma particular, os cuidados paliativos baseiam a sua
atividade clínica multidisciplinar na dignidade da pessoa doente e sua família. Contudo,
enquanto alguns doentes poderão estar a sofrer e morrer com diminuição da sua dignidade,
o conhecimento acerca deste assunto é ainda relativamente deficitário.
Objetivos: Avaliar a dignidade na pessoa com necessidade de cuidados paliativos; Analisar
o efeito da esperança na dignidade da pessoa com necessidade de cuidados paliativos.
Métodos: Estudo correlacional em corte transversal, realizado em contexto paliativo, numa
amostra de 83 participantes (50,6% mulheres), com uma média de 70,95 anos. O instrumento
de recolha de informação ficou constituído por um questionário sociodemográfico, Herth Hope
Index (Herth, 1992) e a Escala de Dignidade do Doente (The Patient Dignity Inventory,
Chochinov et al., 2008).
Resultados: Cerca de 73,8% das mulheres e 73,2% dos homens autopercecionam uma
dignidade positiva (moderada a elevada). Por outro, lado 57,1% das pessoas com elevada
dignidade experienciam uma esperança elevada e quanto maiores os valores da esperança
(que correspondem a uma esperança mais elevada), menores serão os valores da dignidade
(que correspondem a uma dignidade mais elevada). A Esperança revela poder explicativo da
Dignidade (52,9%).
Conclusão: Os resultados obtidos sugerem que a identificação de fatores preditores da
dignidade em pessoas com necessidades paliativas potencia o desenvolver e implementação
de estratégias clínicas para a sua promoção.

Hope has been considered as one of the essential elements in the experience of people with cancer. In the context of terminal illness, when cure is not possible, the focus of hope is of fundamental importance in how sick people are living the period of end of life. In particular, palliative care bases its multidisciplinary clinical activity on the dignity of the sick person and their family. However, while some patients may be suffering and dying with diminished dignity, knowledge about this subject is still relatively deficient. Objectives: Evaluate the dignity in the person in need of palliative care; to analyze the effect of hope in the dignity of the person in need of palliative care. Methods: Correlative study in cross section, held in palliative context, with a sample of 83 participants (50,6% women), with an average of 70,95 years. The data collection tool was composed of a sociodemographic, Herth Hope Index (Herth, 1992) and the Patient Dignity Inventory (Chochinov et al., 2008). Results: About 73,8% of women and 73,2% of men self-perceive a positive dignity (moderate to high). On the other side, 57,1% of people with high dignity experience a high hope and the greater the values of hope (which correspond to a higher hope), the lower will be the values of dignity (which correspond to a higher dignity). Hope has explanatory power in Dignity (52,9%). Conclusion: The results obtained suggest that the identification of predictive factors of dignity in people with palliative needs empowers the development and implementation of clinical strategies for their promotion.
